Healthcare staff wellbeing predictor and outcomes set

The Healthcare staff wellbeing predictor and outcomes set table lists the 45 job demands (physical, psychological, social or organisational aspects requiring sustained effort or skills, such as work pressures) and resources (physical, social or organisational factors that help employees manage demands and achieve goals, such as team cohesion) that experts in the Delphi study agreed were most important for healthcare staff wellbeing. It also includes 12 outcomes that experts agreed are most important to assess in terms of staff wellbeing. They are listed in the table in order of consensus – i.e. those variables that most experts agreed on as important are listed first.

Patient Wellness Questionnaire – Supporting the implementation of Martha’s Rule

In 2017, based on anecdotal evidence that patients and families might have an important role to play detecting and escalating deterioration, we set out to better understand this.

We co-designed a patient wellness questionnaire with patients and staff and tested the extent to which this score was able to predict patient deterioration (Albutt et al, 2017-2022). We found that patients are in a unique position to detect deterioration but they do not always speak up and, when they do, may be ignored.

We went on to evaluate the use of our invention- patient wellness questionnaire in paediatrics and gathered further data to evaluate the predictive utility of the measure (Albutt et al., 2022) In 2023, the NHSE Worry and Concern Collaborative was established, testing and adapting these ideas. We supported this collaborative, offering guidance throughout the programme.

Here we provide guidance on how to implement this as part of Martha’s rule.

Attitudes to Patient Safety

In 2009 we published a paper (Carruthers et al., 2009) reporting on the development and validation of a measure of patient safety attitudes which can distinguish responses between different groups. Although the predictive validity of the measure has not been assessed, it has been used widely across the world to measure patient safety attitudes in a wide range of contexts and for different groups of trainees in healthcare. It has also been used to evaluate undergraduate curricular in patient safety and any changes thereof. Here we provide the original paper (measure is described in the paper). Please note that there is a minor error in the original paper and the item ‘Most medical errors result from careless nurses’ should also be reverse scored before calculating the overall or domain score.

We later (in 2013) updated and extended the measure and this too has been used in a number of studies. Here we provide the original paper (and tool). We have produced scoring guidance for this tool.
